Our Reunification Package provides the complete evidence based process from assessment through to Reunification plan:
Full Child and Family assessment
Parental capacity to change matrix & risk assessment
Parenting Assessment or ParentAssess assessment
6 weeks of direct work with parent/s
6 weeks of direct work with child
Full assessment with outcomes
Reunification plan (if positive outcome)
Further needs plan (if non-positive outcome) for Reunification to be considered again with timeframe
Where there are multiple children/siblings with the same proposed parent/s we offer a separate assessment and plan for each following a needs assessment
